1. Medical Dispute Mediation Application Form
- Personal information/ a compensation claim

2. certification to prove the identity of the person concerned
- person concerned : copy of id card
- representative : power of attorney, id card(both of patient and representative, certification to prove the relationship with patient.

3. Medical Accident Statements
- the truth or assumptions about the medical accident
- a list of damage

4. evidence
- If you prepare all kind of evidence, your medical dispute resolution will proceed more quickly.
（medical records, videos, examination documents, a receipt, earning records of earning, images）
